Term Life Insurance

  • What is Term Life Insurance?

    Term Life insurance pays a benefit in the event of the death of the insured during a specified time period, usually 10 or 20 years. Benefits are paid only if the insured dies while the term of the policy is in effect. It is the easiest and most affordable life insurance to buy. Term Life can be used to cover a family’s major financial responsibilities such as college tuition, funerals and mortgages. Term Life insurance can be used to replace the salary of the primary breadwinner when a family has young children who rely on their parent’s income, or a spouse is unable to pay for the mortgage without the salary of the insured. It can also be used as temporary additional coverage with a Whole Life insurance policy. It differs from Whole Life in that Term Life is for a specified time period, while Whole Life remains in force until the day you die and provides death benefits as well as a cash value accumulation that builds during the life of the policy. In many cases, Whole Life premiums cost three to five times more than Term Life premiums and they typically require a health examination.

  • How do I know if I may need Term Life Insurance?

    Would your death leave a spouse, partner or children financially stranded? If someone you love depends on your salary or income, having Term Life Insurance can ease your mind and keep them protected. You should consider purchasing Life Insurance if you are:

    • Married with children
    • Single but caring for an aging or elderly parent
    • Have a mortgage
    • Own a business and have partners
    • Ready to leave a legacy or provide for a charity
  • Who's eligible for Term Life Insurance?

    Individuals under 65 years of age who are residents of North Carolina and who haven't had a prior Term Life policy with Chesapeake Life within the past 12 months and who qualify for coverage based upon the company’s underwriting requirements are eligible.

  • What is a beneficiary?

    A beneficiary is the person or entity chosen to receive life insurance benefits when the insured person dies. If other than a spouse, sibling or paternal/maternal relative, the person listed must have an insurable interest. Examples would include shared living expenses, shared debt or some other form of shared liability or obligation. The entity could be a business (applicant is an officer or owner of the business); if a charity is listed, there must be proof of previous, consistent giving on the part of the applicant. The benefits are paid to the business or charity, usually through their financial department. You name your beneficiary when you purchase a Life Insurance policy. To change your beneficiary, you must fill out a Beneficiary and Owner Change Request form available online and mail it to:

  • What is an Accidental Death Benefit Rider?

    Accidental Death coverage is an add-on that doubles your selected Term Life insurance death benefit. This additional benefit can be added to the 10- and 20-year Term Life policies, but this benefit may only be selected at the time the Term Life policy is issued. Additional premiums are required, and exclusions and limitations will also apply. Available in North Carolina only. See your Mosaic Group agent for further details.

  • What is a Critical Condition Accelerated Living Benefit Rider?

    The Critical Condition option is an add-on to your Term Life policy that pays a lump sum based on the first time you experience a qualifying event, such as Alzheimer’s Disease, coma, heart attack, life threatening cancer, second- and third-degree burns or stroke. There is a 30-day waiting period for this benefit. This additional benefit can be added to 10-year Term Life policies and may only be selected at the time the Term Life policy is issued. Additional premiums are required, and exclusions and limitations will apply. Available in North Carolina only. See your Mosaic agent for further details.   • What is a Waiver of Premium Rider?

    This additional benefit provides that if the insured person is totally disabled for at least six months, before their 65th birthday, a waiver of premium can be requested for the duration of the disability, as long as the Waiver option is in effect. The Waiver of Premium added benefit can be added to 10- and 20-year Term Life policies, but this benefit may only be selected at the time the initial Term Life policy is purchased. This additional policy must be purchased before the insured reaches his or her 65th birthday. Additional premiums are required, and exclusions and limitations will also apply.   • When can I convert my Term Life policy to a Whole Life policy?

    After the first policy year, a Term Life policy can be converted to a Whole Life policy if the premium is not in default at the earlier of the following: 1) prior to the end of the initial term of 10 or 20 years, and 2) before the insured reaches the age of 70.

  • Are my monthly premiums subject to change?

    Your monthly premiums for Term Life insurance will not increase during the initial term period of 10 or 20 years. If you choose to renew your policy after this initial period, premiums will increase in each subsequent policy year.

  • What are my premium payment options?

    Premiums are typically paid monthly by bank approved draft. Other options to pay are semi-quarterly, quarterly or annually. Payments can be set up for bank draft from your checking or savings account. For assistance, contact Mosaic Group at: (888) 731-2235.

  • How do my beneficiaries file a claim?

    To file a claim the beneficiary must call the The Chesapeake Life Insurance Company Policy Service Member Toll Free Number, 866-215-5343, so the valid beneficiary may be confirmed.

  • Can I access or update my account information online?

    If you wish to make changes to your policy, please contact The Chesapeake Life Insurance Company at: (866) 215-5343.

  • What if I change my mind about my Term Life policy?

    Chesapeake Life wants you to be satisfied with your Term Life policy. Your Term Life insurance policy is a legal contract between you and Chesapeake. Please read it carefully. It is important that you are satisfied with this policy. If you are not satisfied, you may return the policy to Chesapeake or to your agent within 30 days after you receive it. Accident Insurance

  • Why would I need Accident insurance?

    Accidents can happen to anyone at any time. According to the Council for Disability Awareness, a majority of disabling accidents and illnesses aren’t work related and therefore aren’t covered by Workers’ Compensation.1 Having Accident insurance can make the difference in dealing with the financial hardships accidents can cause.

  • What types of treatments or losses are covered?

    Accident insurance pays for losses and treatments such as:

    • Accidental death and/or dismemberment
    • Emergency room treatments
    • Major diagnostic exams
    • Medical appliances
    • Losses related to burns
    • Tendon/ligament injuries
    • Joint dislocation and fractures
    • Physical therapy
    • Hospital admissions
    • Transportation and family lodging in the event of emergencies
    • Various specified losses, such as burns, dislocations, fractures, lacerations, concussions, etc.

    This policy contains a wellness benefit of $60 that can be used once a year for a covered person to undergo one of the many common wellness exams, such as a mammogram or colonoscopy, even if covered by a different policy. It does not coordinate with other coverage. One wellness benefit is paid per policy per year for the primary applicant or one family member (not per person).

    Please note: Accident insurance policies are subject to underwriting and you will be required to answer health questions. The benefits of the policy will not be payable for any loss caused by a pre-existing condition during the first 12 months the policy is in force. After this 12-month period, loss due to such pre-existing conditions will be payable unless specifically excluded from the coverage. This 12-month period is measured from the effective date of the coverage for each covered person.

  • Can family members be covered?

    Yes, spouses and dependent children can be covered under this policy.

  • Who are benefits payable to?

    Benefits are payable in cash directly to the policy owner, regardless of any other coverage you may have.

  • What if I change my mind after I purchase an Accident insurance policy?

    It is important to USAble Life that you are satisfied with this policy and that it meets your insurance needs. Your Accident insurance policy is a legal contract between you and USAble Life. Please read it carefully. It is important that you are satisfied with this policy. If you are not satisfied, you may return the policy to USAble Life or to your Mosaic Group agent within 30 days after you receive it. USAble Life should refund all the premiums you have paid.   • Are there any issue age limits associated with this policy?

    Applicants and spouses applying for coverage must be between the ages of 18 and 64 to be considered.

  • When will my policy be effective?

    Your policy becomes effective on the first of the month following underwriting approval.

  • Is this policy automatically renewable?

    This policy is renewable for life. The policy is renewed month to month with each premium payment that is received.

Critical Illness Insurance

  • What illnesses/conditions are covered?

    Critical Illness insurance underwritten by USAble Life covers:

    • Cancer (if “with cancer” option is selected)
    • Heart Attack
    • Stroke
    • End Stage Renal Disease
    • Amyotrophic Lateral Sclerosis (Lou Gehrig’s Disease)
    • Quadriplegia
    • Major Organ Transplant Surgery
    • Coronary Artery Bypass Surgery
    • Balloon Angioplasty, Stent or Laser Relief Procedure
    • Carcinoma in Situ

  • Can family members be covered?

    Yes, spouses and dependent children can be covered under this policy.

  • Who are benefits payable to?

    Benefits are payable in cash to the policy owner directly, regardless of any other coverage the policy owner may have.

  • I have a pre-existing condition. Will this policy cover that condition?

    Benefits will not be provided during the first 12 months of the policy for a loss caused by a pre-existing condition.

  • I already have a cancer policy. Am I still eligible to purchase a Critical Illness policy?

    Yes, the Critical Illness policy pays benefits regardless of any other coverage you may have. This policy is available for purchase without the cancer coverage, should you wish to protect against all other illnesses in the policy other than cancer.

  • Is there a waiting period?

    This policy contains a 30-day waiting period. Benefits will not be paid for a specified critical illness diagnosed during the first 30 days following any covered persons’ effective date.

  • What if I change my mind after I purchase the policy?

  • I have a high deductible health plan (HDHP) with a health savings account (HSA). Can I purchase Critical Illness coverage?

    If you are currently enrolled in a high deductible health plan (HDHP) with a health savings account (HSA), IRS regulations may limit the types of other health coverage you may purchase. Please see your tax advisor for details before you purchase.
